Black Cohosh
One of the most popular herbal supplements used to alleviate hot flashes is black cohosh. This plant is native to North America and was used by Native Americans for a range of medicinal purposes. The botanical name has been changed in recent years, so some references refer to this plant as Cimicifuga racemosa whereas others use the current term, Actaea racemosa. The famous 19th-century patent medicine promoted for “women’s problems,” Lydia E. Pinkham’s Vegetable Compound, contained black cohosh as a prominent ingredient.

hot flashes.739 Physicians who scoff at herbal remedies appreciate that this research was published in a highly respected peer-reviewed journal, Obstetrics and Gynecology (May 2005). In one trial, Remifemin was compared to a low-dose transdermal estrogen preparation.”O The treatments were equally effective in reducing hot flashes, and neither one was associated with serious side effeCtS.141
Most reviews suggest that the side effects of black cohosh are infrequent and generally mild—mostly digestive upset with some headache or dizziness reported. 42 Just the same, we think that women should discuss this supplement with their doctor. Although recent studies of black cohosh have monitored liver enzymes and found no changes, there are reports of liver toxicity among women taking black cohosh.741 Women who have had hepatitis or other liver problems might want to consider a different approach. For most healthy women, 6 months or so of black cohosh may be worth trying, and it seems fairly safe.744 Unfortunately, black cohosh does not relieve hot flashes that result from breast cancer treatment with tamoxifen.
The Soy Solution
One of the other popular approaches to managing hot flashes is soy. Isoflavones such as genistein and daidzein are phytoestrogens, estrogen-like compounds derived from plants. Even though they are far weaker than the estrogen a woman’s own ovaries make before menopause, it seems logical to use these compounds to ease hot flashes. Studies from cultures like Japan, where women consume soy products such as tofu and ** Soy
The results of studies on soy have been inconsistent. Soy may be consumed as shakes, bars, or more traditional soy foods such as tofu or tempeh. Women who have more hot flashes may derive more benefit.
Side effects: Unpalatable taste, digestive distress Downside: Not advised for women being treated for breast cancer; excess soy may interfere with the production of thyroid hormone

Despite the epidemiological promise, research on soy against hot flashes has been disappointing overall.746.747 One review of many trials, however, found that women who began the studies with frequent hot flashes got more relief from soy foods or isoflavone supplementation than did women who rarely had hot flashes.’ 48 A different systematic review delivered a lessthan-ringing endorsement of the effectiveness of soy preparations against hot flashes, but the authors did conclude that the risks appear to be low. 74″ As the authors of one pilot study concluded, a soy extract (they were testing Phytosoya) appears to reduce hot flashes and night sweats and is probably worth a try if women don’t want to take standard HRT.750
Red Clover
Another source of isoflavones that is marketed for perimenopausal women is red clover (Trifolium pratense). Although this plant is native to North America, its extracts are sold by an Australian firm as Promensit and Rimostil. Most of the research on red clover isoflavones has been funded by the manufacturer Novogen. Nonetheless, the results vary. One small Dutch study found a significant effect, with Promensil reducing hot flashes by 44 percent more than placebo.”‘ But a larger, multicenter trial in the United States found no clinically significant effect on hot flashes with either Promensil or out of the study, and no side effects were significantly more common among those taking red clover extracts than among those on placebo. Analyzing the data for subgroups uncovered a trend toward heavier women getting more benefit from Promensil.153 Overall, however, red clover does not seem very impressive.
Vitamin E
-the North American Menopause Society suggests taking vitamin E along with keeping cool, exercising, not smoking, eating soy isoflavones, and taking black cohosh as inexpensive,.probably helpful nonprescription measures to alleviate hot flashes. Quite a few women have heard that vitamin E can be helpful for this symptom, and some have passed the advice along to their friends.    9
A. I’m 53 and haven’t had a period for a year. I have no problems other than hot sweats.
I recently started taking 400 IU of vitamin E each day and, much to my amazement, it really works. Is there anything I should know about this vitamin?
A. Many women tell us that vitamin E can be helpful for hot flashes. Don’t count on it to protect you against cancer, though. Smokers should probably not take vitamin E supplements, because there is some fear it may actually increase their risk of lung cancer. For others, moderate doses of vitamin E seem safe for short periods of time.

Unfortunately, there appears to be very little research on the effectiveness of vitamin E. Some of the buzz may have come from a study done more than 50 years ago.7-14 Usually we would like to see more recent research to back up a recommendation, but little seems to have been done.— Nonetheless, the North American Menopause Society considers vitamin E, like black cohosh and isoflavones, safe enough to be worth a try.
Vitamin E has performed poorly in several recent studies of cancer prevention, and women who are at particularly high risk of lung cancer—smokers, for example—should probably avoid taking large doses of vitamin E without other antioxidant nutrients. But healthy women planning to take vitamin E for a year or two to ease hot flashes probably will not notice any unpleasant effects. The best product is a combination of natural tocopherols. As with other products, start with the lowest dose available and gradually increase it to find the lowest dose that helps with your symptoms.
It is disappointing that vitamin E has received so little scientific attention over the years. That hasn’t stopped women from trying it, though. Of course, we frequently hear about other interesting nutritional supplements that have worked for a few women but have no scientific evidence to support or refute their effectiveness. If they are otherwise safe, we see no harm in experimenting.
/ can’t help but wonder why most doctors do not treat hot flashes and night sweats with bioflavonoids. As a retired registered nurse, I have found that daily use of bioflavonoids will relieve both the hot flashes and the night sweats. It saves the worry of cancer threats from hormones and is much less expensive. I use Citrus Bioflavonoid Complex, 1,9W milligrams, providing 35 percent hesperidin, 350 milligrams. [Hesperidin is a compound found in citrus fruits that has anti-inflammatory activity.]
At one time, I was hesitant to suggest this to my daughter-in-law because she had been successfully treated for breast cancer When we were visiting and she was showing the lack of sleep front hot flashes and night sweats, I finally bought bioflavonoids for her She often thanks me for the relief that it has given.
Nonhormonal Therapies
For years, women have worried about the possibility that estrogen and progesterone could increase their risk of breast cancer. Women who have already been treated for breast cancer can’t take these hormones safely. As a result, and in light of the negative results from the WHI research, physicians have been looking for other ways to alleviate hot flashes. One approach that seems to help is an antidepressant in the selective serotonin reuptake inhibitor (SSRI) family.

Some doctors now prescribe antidepressants like Paxil (paroxetine) to help women deal with hot flashes. A recent study found that Paxil was significantly better than placebo in reducing the number of hot flashes and easing their intensity. 755 Low-dose Paxil (10 milligrams per day) was also better than placebo in preventing nighttime awakening, presumably due to night sweats. Women in this study did not have to be depressed to get benefit. That also seems to be the case for drugs such as Effexor (venlafaxine).71
Such medications are not without their own drawbacks, though. They can cause sweating, nausea, dry mouth, constipation, insomnia, jitteriness, and sexual problems such as difficulty achieving orgasm. Both Paxil and Effexor also can be difficult to discontinue. Stopping suddenly can lead to odd and distressing symptoms such as dizziness, sensations similar to electric shock, and a peculiar feeling that has been described as “brain sloshing” or “head in a blender.” If antidepressants are taken for hot flashes, they should be taken at the lowest dose that works for the shortest time needed, just like hor-
** Paxil (paroxetine)
Paroxetine eases hot flashes due to menopause or associated with breast cancer drugs such as tamoxifen. The effect on hot flashes is independent of its antidepressant activity. Women found a lower dose easier to tolerate.

Downside: Paxil can be hard to stop taking. You may need your doctor to switch you to a longer-acting drug such as Prozac and then decrease the dose gradually over several weeks or months.
Cost: Approximately $75 to $90 per month move replacement therapy. And, also like HRT, they may need to be tapered off gradually.

Q. 1 have been having hot flashes, night sweats, mood swings, and other change-of-life problems for more than a year. I had hoped these problems would eventually go away, but so far they haven’t.
My doctor wants me to consider Premarin, but I worry about side effects, especially breast cancer. It runs in my family.
The other drug my doctor has suggested is Zoloft. But I am not depressed and don’t want to deal with side effects from that drug either. Do you have information on natural alternatives that might help me withstand hot flashes, night sweats, and interrupted sleep?
A. Black cohosh extract has been recommended for hot flashes. A double-blind study published in Obstetrics and Gynecology (May 2005) showed that the standardized product Remifemin was significantly more effective than placebo.
Another reader shared her experience with a different herb: “I have been using St. John’s wort since discontinuing hormone replacement therapy. It has relieved many symptoms, including sleeplessness, stress, and fits of temper.”

NEURONTI N
Anoffier approach that may help some women who are suffering with hot flashes is an anticonvulsant drug called Neuron-tin (gabapentin). Pilot studies show that 900 milligrams daily of Neurontin reduces both the frequency and intensity of hot
flashes, and that it works better than placebo.757 Because Neurontin, like the SSRI antidepressants, does not have any estrogenic activity, it may be especially helpful for women who have had breast cancer and must avoid HRT. This medicine * Neurontin (gabapentin)
Gabapentin eases hot flashes due to menopause or associated with breast cancer drugs such as tamoxifen. The effect on hot flashes is not related to its anticonvulsant effects.
Side effects: Drowsiness, dizziness, problems with balance, fatigue, swelling of the feet, nausea or vomiting, depression, reduction in white blood cells
Downside: Neurontin should not be discontinued suddenly. You should ask your doctor for help in reducing the dose gradually over several weeks or months.
Cost: Approximately $60 to $150 per month, depending on whether you get a brand-name or generic product
has been associated with some potentially serious adverse reactions, such as a reduction in white blood cells that could leave a woman susceptible to infection. We recommend having a thorough discussion of its likely benefits and risks with the prescribing doctor. This is good advice for any medication, of course, but especially for one being used for an “off-label” condition like this.
Relieving Vaginal Dryness
Hot flashes may be the most obvious symptom of menopause, but for many women, vaginal dryness is just as troublesome. And despite our culture’s greater acceptance of public discussion of sexual issues such as erectile dysfunction, for example, vaginal dryness is often too personal and too embarrassing to bring up. When the WHI highlighted the potential dangers of long-term use of oral estrogen, millions of women stopped their HRT and then searched high and tow lot a personal lubricant that would be safe and effective.

Q. 1 know this is a sensitive issue, but it affects a lot of women. Vaginal dryness is ruining our sex lives. I had breast cancer, so hormones are out. I’m embarrassed to ask my doctor about this.
My husband works long hours, and our opportunities for intimacy are unpredictable. Is there any natural lubricant I could use just at that time?
A. Readers have suggested olive, almond, and vitamin E oils. Some people are allergic to topical vitamin E, though, and it can cause a nasty rash. In addition, using any oil in combination with latex condoms may weaken them. One woman breaks a leaf off her aloe vera plant and uses the slippery gel that oozes out.

For some women, this problem is an issue only for sexual relations, but others find that dryness is uncomfortable throughout the day. Oral HRT generally relieves this symptom along with hot flashes, but just as sudden sweats may return when HRT is stopped, so can vaginal dryness.
Olive Oil
No studies have identified diets, exercises, or other lifestyle approaches that work for vaginal dryness. We. have heard from many women who have found remedies that work for them, however. One of the simplest is olive oil. Other women have found that almond oil has a more pleasant aroma and that it still helps with everyday moisturizing.

If it’s safe to eat, it’s safe enough for vaginal lubrication, too. The oil is applied to the vagina with a fingertip. At first, daily application may provide the best results. Later, lessfreqxiem applications may be eileefive.
Side effects: Some women notice oily spots on their underwear. If this happens, apply less.
Downside: Some women are squeamish about fingertip application. In addition, using any oil in combination with latex condoms can weaken them.
Cost: A few dollars, or less, per month I would like to suggest a natural lubricant that is not greasy but is good for your body. It is pure olive oil. (it can be edible, too.) I have been using olive oil for this purpose for a couple of years. When my doctor did a pelvic exam, he thought I was taking hormones although I am not. I think olive oil has natural compounds to keep women youthful.
There is, unfortunately, no research to show whether any kind of oil applied topically will moisturize vaginal tissues. Our reader got a lot of other people interested in using olive oil for this purpose, though, and some of them contacted us to tell us that it helped. We really don’t know of any hazard to this one except for people who are allergic to olive oil. It doesn’t take very much, so use the best-quality extra-virgin olive oil you can find, or substitute almond oil or another vegetable oil if you
prefer.
Vitamin E
The underground popularity of vitamin E capsules taken orally for hot flashes has apparently inspired some women to try this dietary supplement “off-label.” We don’t think any vitamin manufacturer envisioned women using the contents of a vitamin E capsule for personal lubrication, but some are enthusiastic about it. They prick the capsule and squeeze out the oil for application by hand. Others use the capsule as a vaginal suppository. We caution, though, that some people are sensitive to vitamin E and might develop a rash. In addition,
** Vitamin E
Squeeze the vitamin E oil out of the capsule and apply it to either partner for lubrication “at the moment.” A capsuke may be used as avdiffinal suppository for lasting lubrication.
Side effects: Rash. Test the oil on your inner arm first, to make sure you aren’t sensitive to it.
Downside: Effectiveness has not been scientifically tested. Also, using oil in combination with latex condoms may weaken them so that they aren’t effective.
Cost: Approximately $2 to $5 per month using any oil in combination with latex condoms may weaken them.
Improbable Lubricants
Whoever said “necessity is the mother of invention” must be smiling at the ingenuity of women who have taken common, inexpensive cleansers or moisturizers and tried them to combat vaginal dryness. But some household products have been especially popular. Some years ago, we heard from a couple in their seventies who were using Corn Huskers Lotion, an old-fashioned hand lotion, as a sexual lubricant. Other readers were interested in their experience and tried the product out.

A. 1 suffered for years from vaginal dryness and tried a lot of treatments for it. Then I read about using Corn Huskers Lotion. The results have been incredible! The lotion is inexpensive and works better than a progesterone cream my doctor prescribed. .
A. We’ve heard from others who have found that this old-fashioned hand moisturizer can be helpful for vaginal dryness. Some of the ingredients are identical to those in pricier personal lubricants.

Corn Huskers Lotion contains glycerin, guar gum, and methylparaben, as well as a few other ingredients. It is not dissimilar to K-Y Jelly (glycerin, hyd roxymethylcel I u lose, methylparaben), Astroglide (glycerin, propylene glycol, parabens), or Replens (glycerin, mineral oil, methylparaben). All of these drugstore products are designed specifically as vaginal lubricants. Although they are a bit pricier, such products are certainly worth a try. Corn Huskers is promoted as an oil-free hand treatment lotion. The manufacturer makes no claims regarding this “off-label” use. Do keep in mind that mineral oil, as found in Replens, could compromise latex condoms.
We have also heard from many readers who sing the praises of an old-time facial cleanser for this purpose.
Q. My husband and I have used Albolene as a sexual lubricant since the early 1970s. I’m not sure how we heard about it, but it’s great: odorless, tasteless, slick, but not messy.
It comes in a white tub you can keep by the bed without embarrassment. A 12-ounce jar costs about $11, but a little goes a long way. We’ve purchased five jars in 27 years of marriage. I haven’t seen this anywhere else and wanted to share our secret.

Albolene is a moisturizing cleanser that contains mineral oil, petrolatum, paraffin, ceresin, and beta-carotene. It should not be used with condoms or a diaphragm since petroleum jelly degrades latex.

Albolene is certainly cost-effective. Although it is solid in its container, a small amount applied to the skin soon liquefies and becomes slippery. One drawback, though, is the mineral oil and petrolatum base. These petroleum products will destroy latex, so they must not be used with barrier contraceptives such as condoms or a diaphragm. (The postmenopausal woman may not need to worry about contraception, but many perimenopausal women still need to be vigilant.) I’ve been using a product from New Zealand named Sylk for over a year ear now and find it does a great job in lubricating and relieving pain associated with vaginal dryness during relations.
Quite a few people these days are reluctant to introduce petroleum-based products into their bodies, and they also worry about the effect of such products on latex condoms. People have devised a number of other clever ways to use natural products as lubricants. Some people have found that the gel from an aloe plant is ideal. It certainly is inexpensive! A few drugstore lubricants contain aloe as one of the ingredients, so presumably this is usually well tolerated.

O. My husband and / can’t use K-Y Jelly or any other brand of lubricant we have tried. They make me itch and burn.
We have found, though, that the slimy gel that oozes from an aloe leaf when you break off a piece is a very good lubricant. I hope this will add to your uses of aloe vera and help another couple.
A. This is a most unusual sexual lubricant. Aloe vera get has been used for centuries to help burns heal and ease skin irritation.
Others who would like to try this should test the gel on the inside of the elbow first. If there is no allergic reaction, the slippery texture should make it a surprisingly effective sexual lubricant.
Hormonal Approaches
For decades, when women complained of vaginal dryness as a symptom of menopause, doctors prescribed estrogen, often as a vaginal cream. Most of the time when a vaginal cream or tablet was prescribed, the doctor would point out that it would have local effects and would not be absorbed into the bloodstream. Although vaginal estrogen creams can often help alleviate dryness, the dogma that the estrogen in the cream stays put and doesn’t get into the rest of the body is bogus. The delicate tissue of the vagina is quite efficient at absorbing estrogen and passing it into the bloodstream.758

Q. I have been reluctant to take estrogen because I worry about possible side effects, in particular breast cancer. The worst thing about menopause has been the lack of vaginal lubrication, which makes sex very uncomfortable.
My doctor prescribed a vaginal estrogen cream for this problem. He has assured me that it is locally acting with negligible absorption.
It certainly has helped the vaginal dryness, but my hot flashes have also dropped off considerably. Is this cream getting from the vagina to the rest of my body to control the hot flashes? And if so, what about the risk of breast cancer?
A. Estrogen is easily absorbed from the vaginal lining. In fact, one study of Premarin and Estrace creams published in the Journal of the American Medical Association (December 14, 1979) found that “estrogen vaginal cream preparations, as widely used in clinical practice for their local effects on the vagina(mucosa, actually result in sus126ned high estrogen levels in the systemic circulation.”
We suggest that you discuss your risk factors for breast cancer with your physician. If oral estrogen is inappropriate for you, a cream formulation is not likely to be much safer.
There are certainly times when a vaginal cream or tablet is :appropriate. When other approaches aren’t effective, an estrogen cream often will help. As with oral estrogen, the idea is to use the lowest effective dose for the shortest possible period of time. Frequently the prescription cream will be dispensed with an applicator. Ask the doctor if you should fill the applicator or use less cream than that. It may be possible to apply just a small dab on the tip of your finger and get adequate relief with less overall exposure to estrogen. Topically applied estradiol (a form of estrogen) is available as Estrace cream and Vagifem vaginal tablets.

Q. A couple of years ago vaginal dryness was causing me a lot of discomfort. I am prone to blood clots, so I can’t take oral estrogen.
My doctor prescribed Estring, an estradiol vaginal ring that is inserted every 3 months. It has only 2 milligrams of estrogen and has solved my problem. Please tell others about this approach.
A. Estring has been available in Sweden since 1993 and in the United States since 1996. The 2-milligram dose of estrogen is very low, especially since it is released gradually over 3 months. This approach may solve the problem of vaginal dryness with fewer side effects than oral estrogens.

Estring is a vaginal silicone ring that contains estradiol that is released at a steady, low rate over 3 months, which minimizes fussing. It is placed in the vagina, usually so that it is comfortable or almost unnoticeable.
Side effects: Stomachache, nausea, vaginal discharge, headache, insomnia
Downside: Must pay for 3 months’ treatment up front Cost: $100 to $150 for a 3-month ring There is one more way to apply estrogen topically to the vaginal tissues—with a vaginal ring. This silicone ring is inserted into the vagina, where it releases estradiol at a low but steady rate for 3 months. Like other forms of estrogen, it’s not appropriate for women who have or have had breast cancer. Because the dose at any given time is lower than if a woman were taking estrogen orally, it might be used even by women who are nervous about using estrogen.
Conclusions
Although menopause is a natural process, hot flashes and night sweats can be bothersome. Vaginal dryness also may be uncomfortable. The ideal treatment for these symptoms should be used at the lowest effective dose for the shortest possible period of time, since most symptoms will eventually fade away on their own.
Here is an overview of our recommendations.
•    Keep cool by turning down the thermostat and wearing layers that can be easily removed if you start to sweat. A tall, cool (nonalcoholic) drink is less likely to trigger a hot flash than a steaming cup of coffee.
•    Keep exercising to minimize your hot flashes and help you sleep. Then follow up with relaxation and deep breathing.
O    Try Remifemin. Black cohosh can help with hot flashes if they are not too extreme.
•    Eat moderate amounts of soy products with isoflavones. They may help reduce hot flashes.
•Take vitamin E capsules. Up to 400 IU daily should be safe and might help.
•    If nothing else helps with the hot flashes, try hormone replacement therapy at the lowest effective dose for the shortest possible time. Transdermal estrogen might be worth considering.
•    An antidepressant such as Paxil (paroxetine), Effexor (venlafaxine), or Prozac (fluoxetine) may calm hot flashes even if you are not depressed. Don’t take any of these drugs for longer than you need them; you may need help getting off them.
•7be antiseizure drug Neurontin (gabapentin) may ease hot flashes and does not have the same risks as hormone re-placement therapy. Do not stop this drug suddenly, though, since that could trigger withdrawal symptoms.
•    Vaginal dryness may respond to olive oil, almond oil, or the oil from inside a vitamin E capsule.
•    Corn Huskers Lotion or Albolene offers slippery lubrication for sexual relations.
•    For more natural lubricants, try the gel from a broken aloe vera leaf or Sylk, which has kiwifruit extract. •Estring is the most convenient form of vaginal estrogen. The need for Estring should be reevaluated every 3 or 6 months so you won’t use it for longer than necessary.

The Baby Clinic
When a mother is expecting a child, this is the beginning of an exciting period; a time of ‘expectation,’ followed by the birth and the baby’s first year. It is a special event every time, but particularly with a first child. Parenthood is `born’ along with the child, bringing all sorts of new tasks and experiences with it.
Baby clinics are located at health centres in virtually every town, and have the important task of monitoring the children and helping parents to look after their young children. In addition to ordinary baby clinics, there are also some clinics based on the approach used in anthroposophical medicine. In general, these are linked to anthroposophical medical practices.
All the work of these clinics is concerned with providing preventative care for children from birth to the age of four or five years. In general, this means providing help and support for parents to promote their child’s health.
The doctor and nursing staff at the clinic devote their attention to the physical and psychological development of your child. They ask questions and examine the child to check for certain disorders: for example, growth or psychomotor disorders; disorders or malfunctions of the cardiovascular system, lungs, kidneys and reproductive organs; ear, nose and throat disorders-, disorders of the abdomen, arms and legs; disorders of the teeth, and visual and hearing disorders.
At anthroposophical baby clinics, the constitutional characteristics of the child are also examined, and the personal characteristics and features, which might indicate a particular approach for medical and/or educational measures, are considered. After all, no child develops in accordance with the statistical norm. It is only by examining the individual developmental opportunities and problems of a child that it is possible to give appropriate advice. This is not so much a matter of preventing disorders, but of helping to ensure that the various developmental stages of the child follow each other in an appropriate way.
In addition, the baby clinic is an important place for parents to ask questions, as advice is offered on different subjects, including feeding and growth, upbringing and looking after the baby, physical and psychological development and the issue of inoculations.

The advice on children from birth to one year which you will find in this book is in line with the care provided by anthroposophical baby clinics, and may differ from the advice given by ordinary clinics, and is related to the views held by the doctor and nursing staff regarding the developing child. In Chapter 2 of this book you will find the viewpoints which form the background for the practical advice. This may be helpful when you want to make your own decision in a particular situation. However, this book serves to supplement rather than replace the supervision of the baby clinic. Whichever clinic you choose, it is important that you discuss any concerns you have, and express what you want, at the clinic.
We hope that the various subjects discussed will encourage a conscious approach to parenting and be helpful with regard to understanding, and living with, a developing child.

Pregnancy
Dufing pregnancy, the mother-tobe is in a very special condition, both physically and psychologically. There are all sorts of indications of a reduction in her level of consciousness, which may be manifested by drowsiness, diminished powers of concentration, dizziness, light-headedness and a floating sensation. At a physical level, there is a loss of muscular strength and a loss of tension in all the involuntary muscles. For example, the intestines work less effectively, which can result in constipation. There may also be changes in the action of the kidneys, blood pressure and pulse.
In a way, this whole condition resembles sleep. You could say that a pregnant woman floats between a waking and sleeping state and feels dreamier than she did before. Nevertheless, many women feel very well and active at the same time.
Pregnancy can be divided into three terms, each of roughly three months’ duration.
In the first three months, the woman’s organism has to be ‘transformed’ into a pregnant condition, or, as described above, the organism has to achieve a state between waking and sleeping. The fact that this is an intensive change is clear from the fact that the first three months of pregnancy are usually accompanied by nausea, vomiting and tiredness. It is during these months that the egg is fertilized, becomes embedded in the womb and the foetus starts to develop. It is a relatively vulnerable period of pregnancy in which miscarriages are fairly common (10%). See p.102 folic acid.
The second three-month term is usually the easiest. The pregnant woman becomes used to her condition, can do all sorts of things and does not experience much physical discomfort from the foetus. The foetus has become ‘firmly established,’ as is shown by the small number of problems during this period.
During the third stage, the physical manifestation of the child becomes increasingly clear, with a large stomach being the first sign of this. The discomfort which a pregnant woman experiences when bending down, urinating, feeling full after a meal, being unable to move, run, laugh and sit, reveals that the child has a clear physical presence. The foetus now becomes more vulnerable again. There may be bleeding or even a premature birth. At the end of this period, birth is often experienced as a real release.
When the woman has given birth, all the symptoms of the condition between waking and sleeping gradually disappear again. Only if she breastfeeds will this process be slightly postponed.
An overview of pregnancy reveals that the pregnant woman achieves a condition where she ‘makes way’ for the child to come; in which the child establishes a place on the way to birth. In a way, the expectant mother becomes less ‘earthly,’ while the child becomes increasingly ‘earthbound.’ From this perspective it is, therefore, not surprising that expectant mothers experience moments of contact with their unborn child. After all, both are in an ‘interim state;’ a state between the earthly world and the world that the child is coming from.
If we try to approach the woman’s pregnancy in this manner — taking the idea of ‘making way’ seriously — it is clearly understandable that during pregnancy a woman often finds it difficult to tolerate direct
confrontation with the world around her, and even tries to avoid it altogether. ‘Listening’ to her inner self is the best guideline.
It obviously goes without saying that alcohol and smoking should be avoided because they are known to affect the development and growth of the unborn child, and medicines should only be taken after consultation with the doctor. In addition, a natural, healthy and varied diet is clearly important for both mother and child.
To prepare for breastfeeding, it is a good idea to apply Weleda iris jelly to the nipples every day to prevent the skin cracking during breastfeeding. Stretch marks are largely dependant on genetic factors. However, it makes sense to keep the skin, particularly around the stomach and thighs, supple during pregnancy, by rubbing the skin twice daily with Weleda arnica massage oil. The birth
Experiencing the birth of a child is one of the most intimate experiences in life. Feelings of astonishment, joy, anxiety, fear and fulfilment are experienced to extremes during the birth. It starts with the excitement and anticipation of what will happen when the waters break or the first contractions start. Getting everything ready, the support and help of the midwife during contractions, the constant question of how far the process has advanced — these are all part of the active and busy atmosphere of birth. However, sometimes there are also moments of near serenity and tranquillity; an atmosphere of relaxation, trust and complete surrender to what is to come.
The birth takes place in these recurrent and alternating periods of intense activity and intense tranquillity. Everyone attending the birth finds that a unique atmosphere develops as a result of these alternate emotions, which can go on for many hours. It is an incomparable atmosphere, evoking feelings of deep wonder and awe.
The focal point of everything that is going on is the mother-to-be. She is in touch with the deepest natural forces in her body, and is in danger of being overwhelmed by these natural forces, with an intensity which rarely occurs in life. She may also feel that she does not have the strength to give birth on her own, and may be very grateful to accept the instructions of the obstetric staff, so that the sense of impotence can make way for a sense of trust in the successful end of the birth.
When the cervix is fully dilated, the moment arrives when she can

use all her strength to help the child to be born by pushing it out. Just before this moment, it is quite common for the woman’s consciousness to be almost overwhelmed, and then return quite vivdly with the first push. This stage of pushing the baby out is extremely hard work, even though sometimes it only requires one big contraction.
From the moment that the baby’s head emerges, the atmosphere changes immediately. All attention is focused on the delivery of the rest of the baby, who eventually experiences light, air and gravity for the first time, is placed on its mother’s stomach and swaddled in warm nappies (diapers). If not giving birth at home, you should check with your midwife or consultant to see if it is possible to have a warm cloth to swaddle the baby in. All eyes are on the baby; the noises, movements, eyes and hair. Everyone feels an urge to touch the baby and stroke it.
Then the obstetrician focuses on the last part of the delivery: cutting the umbilical cord and delivering the placenta.
The whole birth is only really complete when the mother has been washed and cleaned up, and is holding the pink, warm, swaddled baby in her arms, and is surrounded by everyone who was present at the birth. The whole spectrum of emotions is experienced, together with a sense of satisfaction, gratitude and respect for the forces that play a role in the birth process.

The birth described above is probably the birth every parent dreams of. And yet, no two births are the same. The life of every person starts with a unique event; the delivery. Some children have a difficult start, for example, if the birth was induced too early, the baby was born prematurely, or if the delivery involved a great deal of medical intervention. In the UK most babies are now born in hospital, but it is possible to discuss your birth plan beforehand with your midwife or consultant.
For parents, the fear about the baby’s health or being overwhelmed by a premature birth can obstruct the feelings of wonder and gratitude described above. Sometimes it may be a while before you can start to love your child in a relaxed way and feel an obvious connection with it, particularly if you feel unsure or anxious. This takes time, so you must try and take the time that is needed. If you were admitted to hospital, you can organize a sort of second birth experience, so that when the baby comes home, you can get used to each other, feel each other, and build up a new life together. Many parents have described that this helped them to recognise the healthy aspects of the child and his lust for life.
Parenthood
The birth of a child is an intense experience for the parents, particularly
the birth of their first child. In fact, it brings about many changes. Before the birth the parents had a relationship with each other, and after the birth they have suddenly become parents and formed a family. Obviously, they still have a relationship, but the partners no longer relate exclusively to each other. In particular, the mother focuses body and soul on her child. After the birth it may be a very long time, sometimes as long as a year, before she feels her old self. Consequently, owing to the new situation in which they find themselves, parents have to redefine the way in which they relate to each other.
This process is extremely demanding because the father and mother are involved with the child in very different ways during pregnancy and birth, and during the initial period after birth. The father may have a tendency to continue his old life with some modifications, while the mother has a deep sense that everything has changed. It may be a while before the partners find a new way of relating to each other on the basis of these two different worlds of experience. It is important to take time for this process and talk about it together from time to time.
What was described above applies particularly for the situation in which mother, father and child(ren) form the family. Where there is a one-parent family from birth, this process will particularly concern the mother.

After the birth, another process starts as well in that all parents discover themselves in a new way. They experience new positive feelings, although they can also have a negative character. A child brings happiness and joy, but there are also moments when irritation reaches unimagined heights.
In the whole range of emotions evoked by a child, feelings of anxiety have a special place, giving rise to questions such as: Am I doing it right’? Will anything happen to my child or me? Will everything be okay?
Every step in the child’s development is another step out into the world. From the age of three, the child even ventures beyond the horizons of the parents; he walks around the block or goes to school for the first time.
Some people are more sensitive to these anxieties than others, but since this anxiety is fruitless — and can really make life difficult for a child — something should be found to counterbalance it. Sometimes gaining an insight into the situation helps to diminish the anxiety, but often this is not enough. In order to tackle the anxiety in a structured way, it may be necessary to work on strengthening the parents’ confidence. Obviously this does not mean blind faith that ‘everything will probably be alright.’ It is not as simple as that. It means that it is possible to work on the confidence about the direction in which the

child is moving, even though unexpected and undesired events may play a role. White anxiety is often ,our own problem,’ confidence can become a strength, which allows the child to flourish; having confidence in someone gives them the strength to grow.
In addition, a child often gives us a new sense of self-awareness: with his behaviour and imitations, he holds up a mirror to his parents. From the age of a few months you will see that a child assimilates the world by imitating it. The child copies everything he encounters, both internally and externally. For parents, this means that what they do and how they do things is important. Whether we do things hastily or with care, whether we do things unwillingly or with joy; all these aspects permeate the actions we perform and are unconsciously assimilated and imitated by the child. This also applies to what we say. Long before the child can understand our words, he will be aware of our intentions. Experiencing this, and occasionally having the things which we do and say, and how we do and say them, reflected by a child will lead to self-awareness, and possibly to a change in our way of being and doing things.
In positive terms, a child stimulates us to develop ourselves as well. There is also a third process. By experiencing the development of a small child and feeling co-responsible for him, it is possible to focus on your own childhood. Some things from your own childhood can lead to the feeling that ‘I want to do things for my children like that as well,’ while at other moments, you feel that ‘I want to spare my children this or that.’ Sometimes this encounter with your own past can be quite intense. It’s good to know that it is not unusual.
Just as we re-examine our own past, we also start to have a different
view of the future; in a sense looking to the future through the child. The future shines through the small child and urges us to determine the structure for that future.
Above, we have highlighted a number of the issues which will confront every parent; the redefinition of the relationship with their partner and other members of the family, a redefinition of themselves, and a new view of the past and the future.

If you are planning to breastfeed, ask about specific breastfeeding support or sessions available in your area prior to the birth (see opposite), as having additional information beforehand is extremely useful and will help you in the first few weeks when you are trying to get breastfeeding established
You should get breastfeeding support while in hospital, both in the delivery suite and on the wards; this may come from a variety of sources, not just from the midwife Many maternity units have

Benefits of breastfeeding
Giving your baby the healthiest start in life
There are several unique advantages to breastfeeding, the main one being that breast milk arrives on demand as the perfect food for your baby. Other benefits of breast milk

include the following:
•    Breast milk protects babies from infection.
•    It reduces the risk of some diseases.
•    Breastfeeding is thought to increase a child’s IQ later in life.
* It can reduce the risk of allergies. It has also been reported that babies who are formula-fed are more likely to have breathing problems such as asthma, and gastric problems
such as colic. Constipation is also more common among formula-fed babies, and hence there are more hospital admissions from bottlefed babies.
* There are greater benefits for the mother if she breastfeeds, such as increased weight loss.
maternity support workers who offer support with breastfeeding. Other units encourage local voluntary breastfeeding support workers to come into hospital to offer women guidance
while they are staying in hospital and then afterwards at home. Some babies will breastfeed well without problems, while others take a little while to learn, so ask for help and
assistance as and when you need it. There are a few tips to remember to help you make a good start: * Skin-to-skin contact at birth is recommended to encourage milk production.
* Good positioning and attachment at the breast is very important Hold your baby close to you and facing your breast, with her head, shoulders, and body in a straight line (see
p.225-229), and make sure that her nose or her top lip is opposite your nipple, so she is able to latch on easily
* Ensure your baby can reach the breast easily, without having to stretch or twist
* Always move your baby towards the breast rather than your breast towards the baby
* Feed your baby on demand, allowing her to feed as often as needed for as long as she wishes * Avoid supplements of water or formula milk unless there is a medical reason for
this that has been fully explained to you.
* Avoid giving your baby bottles or dummies while you are establishing feeding as this can create nipple confusion” as nipples and teats require different sucking techniques.
* Try to relax and enjoy your baby’s feeds
Should I put my baby to the breast as soon as she is handed to me after the birth?
Holding your baby close to you as soon as possible after the birth is recommended, partly to encourage breastfeeding and skin-to-skin contact.

Thinking ahead being practically and emotionally prepared
It’s wise to think about how to prepare for breastfeeding before the birth. * Address your perceptions about breastfeeding and those of your partner and family, and deal with
negative stories This is because some of the problems associated with breastfeeding include embarrassment and a lack of confidence in your ability to produce enough milk.
* Purchase nursing bras and try to attend your antenatal classes, where breastfeeding will be discussed. The National Childbirth Trust (NCT) and La Leche League (LLL) also run
classes on breastfeeding (see p 3 10).
that your baby is close to the breast. Your baby may root for the nipple and some babies will latch on instinctively, while others may just sniff and lick the nipple. Don’t be
worried if your baby does not latch on straight away as babies are born with enough nutrients to last several hours before getting hungry.
What is colostrum?
Colostrum, commonly called the first milk or premilk, is a watery, white/yellow substance produced by the breasts for the first few days. Most women do not notice it until after
the birth, although it starts being produced from as early as 20 weeks in pregnancy and may leak during pregnancy. Although colostrum is not abundant, it has a high
concentration of nutrients. It is high in protein and vitamin K and it also contains antibodies and white blood cells to protect your baby against infection. Colostrum helps
your baby to excrete waste products and lines the stomach with a protective layer that helps fight against foreign substances, such as bacteria, in the body. It is also thought
that colostrum helps to prevent allergies
Help! My breasts are like huge beach balls. Will they stay like this if I carry on breastfeeding?
Between three and six days after birth, your breasts prepare to increase their milk production and may be tender, throbbing, lumpy, and uncomfortably full This is due to the
blood and lymphatic flow to the breasts increasing and a larger volume of milk being produced This is normal, but if milk isn’t effectively removed at each feed, breasts can
become swollen, or engorged. This should be a temporary condition, as long as your baby latches on well and feeds on demand for as long as she needs. Some mothers find it
helpful to massage the breast during feeds to encourage milk flow Wearing a supportive feeding bra and alternating hot and cold flannels can relieve the discomfort
When will my milk come in?
After the birth, your body produces the hormone prolactin, which tells your brain to produce milk, and most women start to produce breast milk between days three and six Which
day your milk comes in may also depend on the type of birth you had, as some studies suggest that milk production is delayed if you have a Caesarean. A delay can also occur if a
woman is on medication, such as insulin, to control blood sugar levels. Also, although this is a natural process, certain factors can affect when, and how much, milk is
produced, such as a woman’s thoughts on breastfeeding and how relaxed she feels, and if a mother is undergoing any stress or anxiety.
There are so many different formula feeds around nowadays - is breast really still the best?
It’s true that there are many types of formula milk, with each one striving to be as close to breast milk as possible. However, there are some nutrients and bacteria-fighting
cells present in breast milk that cannot be artificially produced. Breast milk contains everything a baby needs for at least the first three months As well as promoting the
benefits of their own brand, all formula brands also acknowledge that breastfeeding is the best option.

Establishing breastfeeding
Each mother has a different experience when starting to breastfeed Both you and your baby
will be learning and practising together, which can seem quite strange for something that is supposed to be so natural. Some babies will simply attach themselves onto the nipple
(”latch on”) straight away, while others will take longer and may need help from a midwife Sometimes, the type of labour and birth that you experienced can affect how quickly
you establish breastfeeding.

How should I position myself for feeds? For subsequent feeds, it is worth taking the time to check that you are in a comfortable position (see below, right). It might help if
someone holds your baby while you make yourself comfortable -perhaps with cushions behind your back, or a cushion to lie your baby on.
How should I start the first feed? Many babies begin to breastfeed if left ‘’skin-to-skin”, lying directly on you near your breasts, for about 45 minutes Your baby can smell

How should I position my baby? The key to successful breastfeeding is ensuring that your baby is in the right position and has a good ”latch”. Move your baby so that her nose
is opposite your nipple and “tummy to mummy” (your baby’s tummy is lying across your tummy) in a straight line and held close to you (see below). Wait until she opens her mouth

How do I know if my baby has latched on properly? It is important to make sure that the whole of the nipple and areola are in your baby’s mouth. This enables your baby to get a
good sucking action and prevents your nipples from getting sore or cracked. The baby’s bottom lip should be curled back, and sucking will be long and deep (rather than little
chomping movements). You may also notice that her ears move

I’m expecting twins - can I still breastfeed?
Lots of women successfully breastfeed with twins, although it may take extra planning, as life is easier if both babies adopt the same routine and are fed together. Most women
think that they won’t produce enough milk to satisfy twins; however, milk production works on on a supply and demand basis, so the more your babies suckle, the more milk you
produce You can either fully breastfeed with both babies latching on. or express milk (see p 234-235) and alternate when each baby latches on Expressed milk may be cup-fed to
minimize the risk of a baby taking to a teat and possibly preferring this to the nipple The Twins and Multiple Births Association has plenty of advice on caring for twins (see
p.310).
I’ve heard that it’s harder to breastfeed straight after a Caesarean - is this true?
Women who undergo a Caesarean are likely to be
in more pain than those who have had a vaginal birth, and studies have also shown that postoperative pain
can affect breastfeeding. Also, following major surgery, it’s not easy to move around for a day or so These factors make feeding more challenging initially. However, most
hospitals provide good post-delivery pain relief, which helps women to breastfeed Adopting feeding positions that don’t
put pressure on your stitches also helps (see below). Even if breastfeeding does not happen in the first 24 hours, it is important to allow skin-to-skin contact between you and
your baby as soon as possible.
Will I need a special bra?
It is important that your breast is free during feeding With a normal bra, you would have to remove a garment, so yes, it is advisable to purchase at least two nursing bras.
Nowadays there are lots of attractive bras available. The bra should have a zip or drop-cup fastening to allow one cup at a time to be undone. Ask a trained assistant to measure
you, as a poorly fitted bra can contribute to problems such as mastitis (see p.233) It’s best to wait until 36 weeks before choosing a bra as your breasts continue to grow, The
average increase is around two cup sizes.
Breastfeeding after a Caesarean
Breastfeeding after a Caesarean section can be more challenging than following a vaginal birth as your baby may be sleepy from the effects of the drugs and you will be feeling
uncomfortable from the stitches. If your baby is asleep most of the time, do encourage her to wake for a feed every couple of hours Finding a comfortable position to feed is
important for the let-down reflex.You may find lying on your side facing your baby easier and this is a recommended position after a Caesarean In the days following the
operation, when you are more mobile, you can try feeding sitting up with your baby lying on a pillow to alleviate pressure on your wound.
Breastfeeding is such a struggle. What are we doing wrong?
Although breastfeeding is supposed to be a natural process, for some mothers and babies it can be a challenge. There are a few basic guidelines to help you relax your baby and
get her to latch on properly (see p.228). First, try not to force the nipple into your baby’s mouth. Instead, wait for your baby to lean towards the nipple. For this to happen,
your baby must be turned towards you with her head, shoulders, and body in a straight line (see p,229). Your baby’s lower lip should be below your nipple. To soothe your baby,
you can try stroking her lip with your nipple, or squeeze a few drops of milk onto her lips If your baby wants to feed, she will open her mouth to receive the nipple If so. draw
her closer so that she can latch on across the nipple and around the areola (the darker skin around the nipple) Once she is in the right position, you shouldn’t be able to see
any of your nipple, just a small area of the areola. It should also feel comfortable. Although you shouldn’t force the nipple on your baby you can move her towards the breast so
that her mouth touches the nipple and is encouraged to open wide. Avoid bending forwards, as this can give you backache and may encourage a poor feeding technique.
There are key signs that your baby is properly latched on. These are that the bottom lip is curled back, the chin touches the breast, the mouth is wide open, your areola shows
more above her top lip than under her bottom lip, and the sucking pattern changes to long deep sucks.
How often should I breastfeed my baby?
This is commonly asked by mothers as they feel that the baby should have a routine or pattern. However, it is best not to schedule feeding times and force your baby into a
pattern of, say, every 3-4 hours All babies, but particularly breastfed ones, should be fed on demand. All babies are different and you will soon become familiar with your
baby’s signs of hunger. For example, your baby may ”root”, or search, for the nipple, may not settle, and may make crying or
Troubleshooting
Sore, cracked nipples area common complaint among breastfeeding women and a source of great distress, often leading women to abandon breastfeeding altogether. Knowing what steps
you can take to prevent this happening, or how to alleviate any discomfort, will help to make breastfeeding a more relaxing experience. * Make sure your baby latches on properly
and is removed from the breast gently (see p 228). If your breasts are engorged, expressing some milk first helps your baby to latch on more easily
* Keep your nipples dry between feeds Let the air get to your nipples and use breast pads to soak up leaks of breast milk.
* Relieve sore nipples with a chilled cabbage leaf. You can use a nipple cream if necessary, although most midwives suggest avoiding these if possible.
whimpering sounds. A baby can only hold about 1-2 ounces of milk in their stomach, so some babies may be hungry after an hour, while others may hold out a bit longer. If your
baby dirties a nappy just after a
feed, it is likely that she will become hungry again sooner, usually within an hour of the feed It is also important to allow your baby to feed for as long as possible on each
breast before changing side, to
ensure that she gets the full benefits of the milk
What can I do to help my baby get enough milk?
There are steps you can take to ensure successful breastfeeding and that your baby gets enough milk. * Hold your baby close to you as soon as possible after the birth She will
start to ”root” for your nipple when she is ready to feed.
* Feed your baby as often as she demands in the first few hours and days after the birth This will enable your body to synchronize with your baby’s needs. Feeding on demand in
this way also helps your milk to come in around days 3-5.
* Check that your baby is latched on correctly (see p.228). When your baby is in the correct position, you will both feel comfortable and relaxed. If the baby is not latched on
correctly, it may become painful for you, and you are more likely to stop breastfeeding earlier.
* Allow your baby to feed on one side as long as possible This is because the consistency of breast milk changes during the feed The first part, or foremilk, is lower in fat
compared to the hindmilk The longer she feeds, the more milk you will produce. * Avoid giving your baby a bottle and/or a dummy until feeding is established as this may lead to
nipple confusion. In some cases, a baby may find it hard to latch on, or reject the nipple in favour of a teat. If this continues, your milk production will fall significantly *
Some women believe they should not exercise as it may affect milk production, but this is not the case Studies have revealed that even high-intensity exercise does not affect
breast milk production.
How will I be able to tell that my baby has had enough milk?
Although you can’t measure the exact amount of milk your baby gets, the breasts work on a supply and demand basis, so your body responds to your baby’s sucks and the amount of
milk she takes and produces more according to her needs. Usually, babies feed for at least 10 minutes each feed in the first few days after the birth and you may need to offer
both breasts before she is satisfied. You can tell that your baby is feeding well as her lower jaw will move steadily while she is on the breast. When she is full, she will fall
asleep or release the nipple and be contentedly awake. You should not break the feed, even to change breasts. Your breasts may feel softer and less tense after a feed. Another
sign that your baby has enough milk is the amount of wet and dirty nappies she produces (Breastfed babies tend to have runnier poo than bottlefed ones, see p 242) If you think
that your baby is not satisfied, ask your midwife or health visitor for advice and support before using formula milk.
I get wet patches on my clothes and find breastfeeding so messy. Do you have any advice?
Your breasts leak when they are full and overflow, or when the let-down reflex kicks in, for instance when another baby in the room cries or when you feed from the other breast.
To avoid this, try expressing to stop your breasts becoming too full. Breast pads can help there are disposable and washable ones available. If one breast leaks when your baby
feeds on the other, put a plastic, washable breast shell inside your bra before you start to feed. If the shell
is sterilized, you can save the milk that it collects and freeze it This can be given to your baby at a later date or donated to a milk bank at your local hospital, if you have
one. When you’re out, carry a change of clothes, bra, and breast pads. If you feel a let-down, cross your arms and hug yourself, pressing gently against your breasts, which may
stop the flow. You will probably leak most in the first few weeks of breastfeeding, while you are establishing the right supply for your baby Many women find that the problem
disappears after the first six weeks
Do I have to watch what I eat and drink if I’m breastfeeding?
Yes. Generally, it is important to remember that your baby receives all the nutrition she needs through your breast milk, so having a well-balanced diet is really important (see
p.50). However, there are certain foods and drinks that will affect your baby’s digestion. For example, if you eat lots of fruits, such as grapes and oranges, it can cause loose
stools or diarrhoea in your baby You are advised to avoid high amounts of salt as this can cause your baby to become dehydrated It is also advisable to avoid alcohol Not only
can it make your baby quite sleepy, but there have been studies linking this to cot death
Can all women breastfeed? My mum says she wasn’t able to.
The majority of women are able to breastfeed. You may find that the system of maternity care hindered your mother’s breastfeeding, as there was a time when mothers were told to
feed only every four hours. Learning as much as -you can about breastfeeding in advance makes you more likely to succeed. A common myth is that breast size affects the ability
to feed, but this is not the case. Breast surgery may affect breastfeeding, but even after the most invasive surgery, it is possible that a portion of the original glands and
ducts remain intact. Hopefully you will feel confident enough to give breastfeeding a try.
I want to go back to work six weeks after the birth. Is it worth starting to breastfeed?
Yes, most certainly. Even if you only breastfed for the first week, your baby would benefit from the colostrum. So continuing breastfeeding up to six weeks is good It is
estimated that around 40 per cent of women stop breastfeeding at around six weeks, which may also coincide with the fact that they are no longer receiving visits from a midwife
or health visitor and therefore have a lack of support. Once back at work, you can express your milk, either at work, depending on the facilities, or in the mornings and
evenings at home
What are the benefits of expressing milk?
Expressing breast milk (squeezing milk out of your breasts, see left) enables your baby to receive all the benefits of breastfeeding if you are unable to be with your baby for
every feed. Mothers express their milk for many reasons Some like to give their baby breast milk from a bottle if they are going out when a feed would normally take place, while
others who are going back to work express several feeds’ worth so they can continue to breastfeed their baby, Mothers of premature babies being cared for in a special care baby
unit might express all their baby’s feeds
When can I start expressing?
You can start expressing as soon as is practical after your baby is born. Also, studies have shown that expressing as soon as possible can greatly increase long-term milk
production. For mothers who breastfeed and are returning to work, expressing should start at least a week before so that the baby can get used to receiving the milk from a
bottle or cup. Once you start expressing, if possible, you should express around every three hours, including once in the night when prolactin levels are highest, aiming to
express 6-8 times in a 24-hour period. As breast milk is made on a supply and demand basis, the better your baby feeds, or the more often you express, the more milk you will
make for your baby. An Australian study found that women who express milk are more likely to continue breastfeeding for up to six months.

Providing additional milk supplies
You can express breast milk as soon as you feel ready after the birth, although some women prefer to wait until breastfeeding is established, at around four weeks. Expressing
milk means your partner can start to help with feeds and you may be able to get out for periods.
How is it done? Most women use a pump to express their milk There are many different types available, ranging from manual to electric ones. The other way to express your milk is
manually. To do this, support your breast with one hand, making a c-shape towards the back of your breast and gently squeeze in a downward motion, moving towards the nipple;
stop, and then repeat until you have enough milk You will soon learn where the
best place is to put your finger and thumb. Sometimes it is difficult to get a”let-down reflex” when you are expressing – try thinking of your baby and you should soon be making lots of milk.
How should breast milk be stored? It is important that you put the expressed breast milk into a sterile bottle liner or a sterile bottle. This can then be stored in the fridge
for 24 hours, or in the freezer for up to three months Label each bottle or container with your name (if your baby will be with others at a nursery), and the date and time you
expressed it. To defrost the milk, warm it gently in a bowl of hot water – don’t use your microwave Do not keep milk in the fridge door, as the temperature fluctuates.

Twins and multiple births

We are expecting twins following IVF treatment. How will we cope?
Although finding out that you will be the parent of two babies rather than one can be a shock, the initial surprise will settle and you will soon start to get used to the idea There are many associations that offer information and support to parents of twins, as well as companies that make products for parents of two or more children (see p.310) Your midwife and obstetrician will offer information and support and may put you in touch with local multiple birth support groups You will also be invited for more regular antenatal appointments and scans than if you were having just one baby to keep an eye on the growth of your babies.
As with all multiple births, there are no additional financial benefits if you are having twins, although you may receive more of certain benefits that are dependent on income (see below).
We’re having triplets. Help! My wife is over the moon, but I feel numb. Where can we get advice?
As having triplets is relatively rare - only 149 sets
of triplets were born in the UK in 2006 - the majority of information and support for couples does relate to having twins. However, more and more research is being carried out into how to help and support parents having more than two children
Your midwife and obstetrician will be great sources of information and will be able to put -you in touch with other parents of multiple-birth children. There are also several organizations that offer support and information for parents having a multiple birth (see p.310). As you and your wife learn more about having triplets, your anxiety will hopefully start to ease
Will we receive any additional financial or practical support
as we’re having more than one baby?
Unfortunately, there are no financial benefits available to all parents having twins or multiple births. However, there are some benefits that are dependent on your income, some of which you may be able to claim per baby One of these is the Child Tax Credit, made up of three elements: a family element: an amount payable per child dependent on your joint income; and a baby element of £545 if you have cne or more children under a year old This credit is the focus of the Twins and Multiple Birth Association’s current campaign, as they feel strongly that the baby element should be paid per baby, so that a family with newborn triplets would be entitled to £1635. The Sure Start Maternity Grant, a payment of £500, is payable per baby so if you are entitled you would be able to claim £ 1500 for triplets. This must be claimed within three months of the birth so it is important to apply as soon as possible For practical support, it is worth finding out about Home Start schemes in your area. Home Start is a charity that provides trained volunteers to lend support at home. Each scheme is locally based, managed, and run by individual communities, supporting families in that community.

Does taking folic acid increase the incidence of twins?
There has been some debate and conflicting studies about whether taking folic acid pre-conceptually could increase the chance of having twins. A study in Sweden in the 1990s found a higher incidence of multiple births among women taking folic acid. However, this could be attributed to other factors, such as a greater number of women undergoing fertility treatment, which carries an increased probability of twins. Also, subsequent studies have refuted these findings; in 2003, the medical journal The Lancet reported on a large-scale study in China that found there was no significant difference in the number of women who had taken folic acid carrying twins.
Are all same-sex twins identical?
No. Whether or not twins are identical depends on how they were conceived, not on what sex they are (see above). While identical twins are obviously the same sex, non-identical same-sex twins are as similar or different as any other non-twin siblings.
How likely is it that our twins will be identical?
One in 80 pregnant women carries twins and one-third of twins are identical Although there are factors that make you more likely to have non-identical twins, such as a family history of twins or being over 35, having identical twins is not an inherited trait and there are no other factors that make this more likely.
Will I know before the birth if they are identical?
The term “zygosity determination” means finding out whether twins, triplets, or more are identical (monozygotic) or non-identical (dizygotic or fraternal). It is natural for parents to want to learn all about their babies, and with twins this includes their zygosity As well as for reasons of natural curiosity, knowing whether twins are identical can help parents to determine the chance of having a multiple pregnancy again, and also has implications on care during pregnancy, as identical twins, especially if they share a placenta, are higher risk, and so the pregnancy may be more closely monitored.
In two-thirds of cases, the placenta provides the answer as to whether twins are identical. If the babies have a single amniotic sac surrounded by one outer protective membrane, known as the chorion, they are monozygotic However, one-third of identical twins whose egg split early, before the placenta started to form, have two chorions with either a fused placenta, where two placentas grow together, or two separate placentas. These placentas are hard to distinguish from those of dizygotic twins
We don’t know if our twins are identical. Will it be obvious after the birth?
In a third of cases, twins are different sexes and therefore obviously non-identical In same-sex twins by the time the children are around two
years old their’ zygosity” is usually quite clear from their physical features Before this, there are many indications as to whether twins are identical such as the colour of their hair and eyes, the shape of their ears, the eruption and formation of teeth, the shape of the hands and feet, and the pattern of growth
If there is doubt as to whether twins are identical, the most accurate way to determine zygosity is by the DNA probe method when tiny amounts of DNA are collected with a swab from inside each twin’s mouth. A laboratory examines specific markers present in the DNA and 12 diagnostic targets are compared. Although non-identical twins may share five marker patterns by chance, monozygotic or identical, twins will have the same pattern for all 12 markers
Will I love one twin more than the other?
Although this can be a concern, it is more likely to be the case that rather than favour one child over the other, a parent gives more love and attention to the baby who needs it most at that particular time
It is also possible that the strain of having two new babies in the house may increase the likelihood of delayed bonding, although this can also happen_ the birth has been traumatic if the mother or indeed the father is exhausted: or if one baby has taken time to establish feeding, or is more fractious than the other This does not mean that bonding will not take
Am I likely to have a normal birth?
Although many women having twins have normal deliveries, the rate of Caesareans is increased with twin births With one baby the Caesarean rate is around 25 per cent in the UK; with twins, the rate is closer to 50-60 per cent which also means that 40-50 per cent of twins are delivered vaginally. Triplets and above are generally delivered by Caesarean in the UK and Europe Whether or riot twins are born vaginally depends on their position in the womb ~ whether one or both twins is head down (see p 133).
There may be an indication as to the type of birth in pregnancy as women with twins are usually scanned to check the position of the babies near to term, at around 27-34 weeks.

In every family, there are bound to be ebbs and flows of love between parents and children, which is normal and not a cause for concern When a parent has two children born at different times, that parent may love one child differently to the other, but this does not mean that the love a parent has for one child is to the detriment of the other.
Will the side effects of pregnancy be much worse with a multiple pregnancy?
Although in some cases the side effects of pregnancy may be the same when you are expecting two or more babies, the likelihood is that many pregnancy symptoms will be exaggerated Symptoms such as morning sickness, fatigue or exhaustion, disturbed sleep and swollen hands and feet are often worse with a multiple pregnancy Unfortunately, women with multiple pregnancies also tend to suffer more from varicose veins (see p.86) In addition to these increased side effects, weight gain is greater and more rapid for mothers carrying more than one baby and the uterus measurement is often increased for the gestational age This extra weight and size caused by carrying two or more babies may also cause more constipation haemorrhoids (piles), urinary tract infections, and vaginal thrush infections.
Although there may be more exaggerated symptoms with a multiple pregnancy the majority of these problems can be monitored by your midwife or doctor, and they may be able to offer advice and treatment to ease these symptoms.
Will my weight gain be much greater than for someone who is having just one baby?
Mothers of twins or triplet pregnancies are likely to gain more weight than women having one baby. Indeed, in the first trimester, rapid weight gain may be an indicator of a multiple pregnancy The increased blood volume and size of the uterus, as
well as each baby’s weight, possibly two placentas, and the amniotic fluid for each baby, will continue this pattern of greater weight gain during pregnancy
Although on average a woman having a multiple pregnancy is likely to put on around I Okg (221b) or more than a woman having one baby, this is not double the weight gain If you are having twins, you should raise your calorie intake by only 500 calories per day in the last trimester, compared to 200 calories more for a single pregnancy
I’m only 24 weeks, expecting twins, and already I’ve got high blood pressure. What can I do?
Unfortunately high blood pressure is more likely to start, or worsen if you already have the condition, in a twin pregnancy as the rates of pregnancy-induced hypertension (PIH) and pre-eclampsia (see p 89) are increased in multiple pregnancies
There is little that can be done to prevent PIH General lifestyle changes, such as reducing your salt intake, avoiding alcohol and tobacco, taking gentle, regular exercise, and getting enough rest, are thought to help. You should also ensure that you attend all your antenatal appointments and contact your midwife or doctor if you experience headaches or visual disturbances such as flashing lights or there is reduced movement from your baby
What can go wrong if I have a vaginal delivery?
If both twins are head down, a vaginal birth is usually possible. Sometimes, the first twin may be head down and born vaginally, but the second twin may be breech Sometimes, the second twin will turn and be head down after the birth of the first twin, and you are then more likely to deliver both twins vaginally Studies suggest that there has been a significant increase in combined vaginal-Caesarean births of twins and a decrease in vaginal only births, which may be due to the fact that there is a greater willingness nowadays to allow women carrying twins to try for a vaginal delivery, which also increases the likelihood of this scenario. If you have a vaginal delivery, there is a greater chance of one or both twins having an assisted delivery by vacuum extraction or forceps (see p.202), either because one or both twins is positioned in a tricky -way, for example facing the mother’s back, or because the labour may be longer and weaker because of the amount of work involved in pushing two babies out, which means that the mother is therefore likely to be more tired and needing help at the end of labour.
Why might the doctors decide to deliver my twins by Caesarean section?
An elective Caesarean (see p.206) might be recommended for a twin delivery for several reasons, but ultimately it is your decision The optimum time for delivering any baby is at term (37-40 weeks’ gestation) and this remains the case for delivering twins as they may well be smaller than a singleton baby, having had to share your supply of nutrients However, if one or both of the babies are compromised, possibly due to twin-totwin transfusion syndrome (see p 134) or raised
blood pressure in pregnancy there may be a need to deliver the babies preterm.
Many units recommend a Caesarean for a breech baby where the baby is bottom down inside the womb, because there are more risks associated with a breech vaginal delivery In a twin pregnancy if the first baby is breech, this puts the second twin at risk too Also. if the first twin is breech and the second is head first (cephalic), a Caesarean is recommended due to the rare complication of ”locked” twins, when the babies’ chins get locked together
If both babies are head down and appear to be thriving, many maternity units will encourage a normal delivery Your doctor and midwife will discuss this with you nearer the delivery time.
Will my triplets need to be delivered before 40 weeks?
Yes, it is very likely that your triplets will be delivered before 40 weeks. Although most twins are born at around 37 weeks, which is considered to be a term pregnancy it is rare for triplets to reach term, and most are delivered at around 32-36 weeks’ gestation
As a woman’s body is designed to carry one infant at a time, carrying more than one increases the risks for both mother and babies, and the decision to deliver your triplets will be taken when one or more of the babies is not coping well. To improve the chances of a good outcome, get plenty of rest and eat a healthy diet (see p.50) Although premature deliveries do carry a risk to the infant, if the baby’s wellbeing is compromised an early delivery is necessary. If you go into premature labour, you may be given medication (see p.162) to try to stop labour for long enough to administer steroids, which will help to mature the babies’ lungs before delivery -as long as this does not put the babies at risk
How likely is it that my twins will have a lower than average birth weight?
Over 40 per cent of twins are born with a lower than average birth weight, which is mainly due to the fact that they are born earlier than singleton babies.
Do twins run out of room to turn in the womb?
It does tend to be the case that, in the third trimester, twins find a position and settle there at an earlier stage of pregnancy than if there was just one baby. Generally, with twin pregnancies there seems to be a lot less movement in presentation from about 32-34 weeks. However, how your twins are likely to be delivered depends largely on the direction that the twin who is lowest in the pelvis is facing. If this twin is head down, then a vaginal delivery should be possible and the second twin may be able to be gently coaxed into a favourable position, or may need to have an assisted delivery (see p 202)
I’ve been told that one baby isn’t developing as well as the other.What will the doctors do?
Although it is common for twins to grow at a different rate in the womb, if there is a significant difference in size, it may be that one baby is getting a greater proportion of the nutrients than the other. It is important to check that your babies are developing in line with their gestational age. It is not unusual for some babies to grow slowly and then accelerate later on, which is not a concern if it’s within the accepted range of growth for their gestational age. However, if your midwife or doctor is concerned about the development of one baby, they will probably refer you to a fetal medicine specialist: an obstetrician with additional training in caring for the unborn baby He or she may do blood tests and perform an ultrasound to assess the growth of each baby and investigate why there is a difference.
You may continue to have additional scans, known as growth scans, which will help the doctor to assess if one baby is small or growing slowly These usually start around 26-28 weeks and continue every 2-4
weeks until your babies are due to be delivered They look at a number of areas including the head, abdomen, and thigh bone measurements; the amount of amniotic fluid around the babies; the babies’ levels of activity; the blood flow in the umbilical cord; and the position of the placentas. Your doctor should
explain the findings of the scans and if there is a concern you will be closely monitored.
What is twin-to-twin transfusion syndrome?
This is a rare but serious condition that occurs only in identical twins who share a placenta. It is caused when there is an abnormal blood supply and a blood vessel directly connects the twins. One twin pumps blood around his own body and that of his twin and, as a result, he does not grow properly An early delivery is usually needed to save the smaller twin.
Am I likely to lose one or more of my babies?
There are increased risks for both mother and babies associated with multiple pregnancies and sadly there are occasions when one or more of the babies dies in the womb This occurs in around 2 5-5 per cent of twin pregnancies. In some circumstances, for example if there is a fetal abnormality in one twin such as a heart defect, the doctor may suggest that one or more of the babies is terminated in the very early weeks to allow the normal healthy development of the other baby or babies. However, many doctors believe that this is unnecessary as the procedure itself carries the risk of losing all the babies.
Although incredibly hard, this is ultimately your decision so you should spend time discussing the options with your doctor.
Unfortunately the death of a baby in a twin pregnancy can sometimes cause problems for the surviving twin, although the degree and type of problem depends on whether the twins were identical or non-identical. If the twins were identical, the doctors will want to assess whether it was a monochorionic pregnancy (in which the twins share the same placenta) or a dichorionic pregnancy (in which they have a different placenta). This is because, when the placenta is shared, there is a 30 per cent risk of death or a neurological problem to the surviving twin if the other dies, whereas if there are two placentas, there is a lower risk of 5-10 per cent, of death or disability occurring in the surviving twin.

 

 

 

The position of twins
Twins can lie in a variety of positions in the uterus and these positions can determine how your baby will be born One baby will always be lower than the other one, and this baby will be known as the first baby - it is closer to the birth canal and will
generally be born first.
What are the possible positions? Babies can be in the head down position (cephalic) or buttocks or feet first (breech). Occasionally a baby may be lying across you diagonally or horizontally (transverse) Twins can lie in any combination including: cephalic-cephalic, cephalic-breech, breech-breech, breech-cephalic These positions can change throughout the pregnancy, As with a singleton pregnancy (one baby), once the presenting baby nearer to the cervix goes down into the pelvis, it will stay in that position ready for birth.
Can I have a vaginal birth? When both babies are in a cephalic position you may be offered the chance to try for a normal labour and vaginal birth Sometimes, the first baby is cephalic and the second twin is in a breech position. If this is the case, your obstetrician may suggest that you have a Caesarean from the outset, or may suggest that you have a vaginal birth with the doctor assisting the birth of the second twin with forceps or ventouse (see p 202) if necessary You can certainly be party to these discussions and it’s important to share your feelings about the birth and birth choices. If the first baby is breech and the second baby is cephalic, then it is highly likely that your doctor will recommend that you have a Caesarean delivery. If both your babies are in the breech position you will almost certainly need a Caesarean, as is the case if both babies are lying across you in the transverse position

 

How are twins conceived?
Identical ”monozygotic” twins are produced when a single egg is fertilized by a single sperm, and the egg then splits into two. The babies may share the membranous, or amniotic, sac that surrounds them in the uterus Depending on when the egg splits, they may also share a placenta. Identical twins, therefore, are the same sex and look almost completely alike as they share the same genetic makeup. Non-identical, or ”dizygotic”, twins result when two eggs are fertilized by separate sperm at the same time and each therefore has its own individual genetic makeup Each fetus also has its own amniotic sac and placenta.

Keeping active in pregnancy
I regularly go to the gym. I’ve just found out I’m pregnant -can I still go?

Many forms of exercise are safe during pregnancy Regular exercise keeps you fit and healthy, so if you currently exercise then it’s fine to carry on as before Although you can continue to take part in most activities during the first trimester of your pregnancy, you may need to stop more vigorous exercise as your pregnancy continues Do tell your fitness instructor that you are pregnant, so they can tailor your programme accordingly — pregnancy is not the time to break records or go for personal best! Ideal exercise gets your heart pumping, keeps you supple, manages weight gain, and prepares your muscles for the hard work of labour and delivery without causing undue physical stress for you or your baby.
Benefits of exercise

There is no doubt that exercising during pregnancy offers numerous benefits to both mother and baby.
* Regular exercise increases flexibility and suppleness, which will benefit you in labour, * Aerobic exercise, such as swimming, increases stamina, improving blood circulation and preparing you for labour. * Exercise releases endorphins, the body’s natural painkillers, helping you to relax and lifting your mood
•    Exercise keeps backache at bay.
•    An exercise regime will help you to recover more quickly after the birth
Being active during your pregnancy can also reduce the physical discomforts of backache, constipation, fatigue, and swelling, as well as improve your mood and even help you to sleep more soundly. The Royal College of Obstetricians and Gynaecologists (RCOG) states that weight-bearing exercise in pregnancy can reduce the length of labour and may decrease delivery complications. So continue if you can.
Other forms of exercise recommended in pregnancy include swimming, walking, aquanatal classes, yoga, and pilates, as these are not high impact so are less likely to injure your joints.
What’s the best type of exercise during the third trimester?
Swimming is an excellent form of exercise and can be maintained safely throughout pregnancy
It improves circulation, increases muscle tone and strength, builds endurance, and is favoured in late pregnancy as it makes you feel almost weightless. Many women find aquanatal classes enjoyable
exercising while standing in water is gentle on the joints and helps reduce swelling in the legs, common in late pregnancy Aquanatal classes may be run either by a local midwife or by an exercise teacher trained to teach pregnant women.
Walking is a good form of exercise for this later stage of pregnancy as it keeps you fit without jarring your knees and ankles. Take some water to drink to avoid dehydration Yoga and pilates are good if you can find a registered practitioner experienced in dealing with pregnant women. Yoga teaches breathing and relaxation techniques that can help with the demands of labour and birth. Many pilates exercises are done in a ”hands and knees” position, which is ideal for pregnancy as it takes stress off the back and pelvis and, towards the end of pregnancy, can help to position your baby ready for delivery.

I’ve had a previous miscarriage - should I avoid all kinds of exercise?
Many doctors feel that it is best to avoid all but the gentlest forms of exercise in the first 12-16 weeks of pregnancy if you have had two or more miscarriages, or have had vaginal bleeding during this pregnancy
I’m not terribly fit, but would like to start an exercise regime - any advice?
If you are unused to exercise, then moderate activities, such as walking and swimming, would probably be best for you and beneficial for your baby, whereas starting a new competitive sport or vigorous exercise programme would not be ideal Your body is already undergoing huge changes with your heart, lungs, kidneys, and virtually every other major body organ beginning to work much harder. Also, the pregnancy hormones progesterone and relaxin are softening the muscles and ligaments, so soft tissue injuries, back injuries, and abdominal strain become more likely, especially if you haven’t exercised much before. Contact sports, vigorous team sports, and activities like diving and gymnastics carry the further risk of direct injury to your abdomen and uterus - especially as your uterus grows and rises out of your pelvis.
Is it safe to go jogging when you’re pregnant?
Exercise is recommended in pregnancy to improve your circulation and energy levels, boost the immune system, and increase your stamina for labour. Although low-impact activities, such as walking, swimming, and gentle toning and stretching, are ideal, if you are used to jogging and your pregnancy is straightforward, it is fine to continue in pregnancy. However, it is not advisable to take up jogging for the first time now, particularly as there is a risk of falling and hurting your abdomen, and you should avoid jogging if you have a high-risk or multiple pregnancy Other sports and exercises to avoid include gymnastics, horse riding, skiing, and squash.

When should I start doing pelvic floor exercises?
Pelvic floor exercises (see above) can be started at any stage of your pregnancy, but the earlier you start them the better These exercises strengthen the pelvic floor, which is the network of muscles that support the bladder, womb, and bowel. Strengthening these muscles helps to reduce the risk of leaking urine while coughing or sneezing, known as stress incontinence It is important that you know how to do these exercises and practise them regularly throughout your pregnancy
As well as practising the exercises shown above, another way to exercise your pelvic floor muscles is by inserting a finger into your vagina and tightening the muscles around it.

I’m very desk-bound in my job - is it dangerous to sit for long periods of time?
During pregnancy, your circulation slows down and if you sit for long periods of time with the lower leg vertical, it can make it hard for blood to travel upwards. Although this may increase the risk of a blood clot, known as deep vein thrombosis (DVT), sitting for long periods in itself is unlikely to cause a clot Your degree of risk also depends on your level of activity at other times. Exercise is the best way to minimize the risk of a blood clot and taking a brisk daily walk is ideal as it exercises your legs. There are also simple measures you can take while at work to reduce the risk of developing a clot. Try ankle movements every hour, get up and walk around every 3-4 hours, take the stairs rather than the lift, and walk over to see a colleague rather than email.
If you are especially concerned, talk to your midwife or doctor about wearing special stockings that are designed to improve circulation However, it is important that you get the right size, as stockings that are too tight can add to the problem.

I’ve been getting lower back pain - could it be due to bad posture? I’m eight months pregnant.
In a recent review of current research, more than two thirds of pregnant women reported back pain. This pain increased with advancing pregnancy interfering with work, daily activities, and sleep. Lower back pain is caused by the forward pull of the growing abdomen, so as your baby increases in size and gestation, the strain on your back is greater So although bad posture may not be the sole cause adopting a good posture is important to reduce the strain Gentle exercise also helps to reduce the pain, and water aerobics is particularly beneficial.
Some women use a Transcutaneous Electrical Nerve Stimulation (TENS) machine in late pregnancy (see p.175), which helps to block the pain nerve impulses to the brain and stimulates the release of natural painkillers called endorphins. Other tips for lower back pain include a warm hand massage using a base oil a warm deep bath, and using cushions to support you when relaxing and in bed
I’m seven months’ pregnant now and quite big. Should I adapt my swimming style?
You may find that as you get very large towards the end of pregnancy, you need to alternate your swimming style to find the one that is most comfortable for you. Apart from this, a low-impact activity like swimming is ideal as the water provides resistance, there is a low risk of injury and the mass of water relieves pressure on the abdomen and helps to ease lower back pain.
My midwife is running aquanatal classes at our local pool - are they safe?
Yes, low-impact activities such as aquanatal classes are fine in pregnancy. You are fortunate to have this facility as not all areas are able to provide classes. The trainer or midwife conducting the sessions
should be able to advise you about the range of movement recommended to minimize any risk.

We love going clubbing; will the loud music be OK for my baby?
There is evidence to suggest that babies can hear
in the womb from about 16-20 weeks. However, your baby is protected by the amniotic fluid surrounding him, so most noises do not affect him. The ears of a fetus are often full of a protective greasy coating produced by the skin, known as vernix, so external loud noises would be muffled by the time they reach your baby. Your baby is most likely to respond to your reaction to loud music rather than the music itself
There is a study that suggests that constant or regular exposure to noise can increase the risk of a small-for-dates baby, meaning your baby’s growth is smaller than expected for his gestation. However, it is more likely that it is the environment and its effect on the mother that contributes to the baby’s weight rather than the actual noise. Too much clubbing may mean you are getting too little rest and you may be drinking more alcohol than you should. You should
probably consider whether you are getting enough quality rest and ensure that you are reducing or stopping your alcohol intake, as this is more harmful to your baby than loud music.
We like walking, but should I cut down on the number of miles now that I’m pregnant?
Walking is ideal in pregnancy as it is low-impact exercise and can be maintained throughout your pregnancy If you plan to continue lengthy walks and like to walk briskly, try combining this with a slower, more leisurely pace. It’s important to control your body temperature so that you don’t overheat and feel uncomfortable To do this, drink plenty of water to avoid dehydration and wear layers that you can take on and off as required. As your tummy grows, you may find hill climbing causes physical instability, as may trekking over uneven terrain, so stick to more level paths. If you find yourself getting breathless, take frequent breaks.

Antenatal yoga
Practising yoga in pregnancy is hugely beneficial. As well as strengthening and toning muscles, which will help you in labour and birth, yoga aims to bring about a greater awareness of your breathing rhythms, providing a perfect relaxation tool in pregnancy and preparing you to breathe through the contractions. Find an accredited teacher experienced in teaching pregnant women or attend an antenatal class.

Exercise in late pregnancy

Towards the end of pregnancy, you will inevitably slow down, but you may not want to stop altogether! There are sensible ways to modify your exercise regime to ensure you stay safe and fit. * In later pregnancy, avoid exercises that involve sudden movement, such as tennis, as your balance is less steady now. * Swimming is perfect in late pregnancy and aids relaxation.You may need to modify your stroke and enjoy more floating. * Reduce the intensity and length of your workout to avoid exhaustion.

Pelvic floor exercises Strengthening the muscles that support pelvic organs
Learning how to exercise your pelvic floor muscles is vital in pregnancy to help you avoid stress incontinence (leaking urine). This discreet exercise can be practised any time. Pelvic floor exercises involve squeezing your buttocks in and pulling in your tummy muscles, then holding for 5 seconds and releasing. Repeat this 5-6 times several times a day. You could imagine your pelvic floor going up like a lift, contracting your muscles a little more at each floor.

Although exercise is highly recommended during pregnancy, this is a time when you may have to moderate your usual programme, especially as you get bigger, and avoid types of exercise or situations that may put you or your baby at risk.
What safety precautions should I take? If you are taking up exercise during pregnancy, be sensible about which type of exercise you choose. Avoid any type of exercise that is too strenuous and opt for low-impact activities, such as walking and swimming. Always do warm-up stretches before exercising and build up your stamina and fitness gradually. This is especially important as hormones in pregnancy relax joints and ligaments
in preparation for labour (see opposite), which means that you are more susceptible to injury. Avoid exercising in very hot conditions as this may be harmful to the baby; in hot months, exercise early in the morning or indoors Also, avoid exercising near traffic as you are more likely to be affected by pollution while exercising. Your centre of gravity changes in pregnancy, so avoid high-impact, fast-moving sports, such as tennis.
Should I stop exercising at any time?You should stop exercising straight away if you feel dizzy or short of breath; if you feel that you are overheating; if you are experiencing pain in your back or pelvis; or if you feel exhausted.

Taking Medicines in Pregnancy

What is safe to take?
The advice to pregnant women is to avoid taking any medicines in pregnancy if at all possible. If you do need to take medication, check with your midwife or doctor first, or ask your pharmacist for information on over-the-counter drugs. The list below offers some guidance
Antiemetics: For women with severe morning sickness, an antiemetic drug may be suggested Your doctor will recommend one that is safe to take in pregnancy
Antihistamines: Most of these should be avoided in pregnancy. If you have hay fever, try to avoid known triggers and allergens or talk to your doctor about safe medications in pregnancy
Painkillers: If natural remedies, such as a head massage to relieve a headache, or a warm bath to ease backache, don’t work, then paracetamol is generally considered safe for short-term use in pregnancy although it should be avoided if possible. Ibuprofen should be avoided altogether. as should aspirin (unless specifically prescribed by your doctor)
Antibiotics: There are antibiotics that are safe for use in pregnancy. Penicillin-based ones are usually prescribed, or if you are allergic to these there are other safe alternatives The following ones should be avoided in pregnancy!
* Tetracylines can affect the development of a baby’s bones and teeth and may cause discolouration of the teeth.
* Streptomycin can cause damage to the ears of the growing fetus and result in hearing loss and so should be avoided in pregnancy
* Sulphonamides: These cause jaundice in the baby and should not be given in pregnancy
Laxatives: If you are suffering with constipation, try natural dietary remedies first, such as eating lots of fibre and drinking plenty of fluids If these don’t work, then over-the-counter laxatives are
safe to take in pregnancy. Ones that contain bulking agents are the best.
Antacids: Heartburn is a common problem in late pregnancy due to the pressure of the baby on the stomach. Antacids are generally safe to take, but avoid sodium bicarbonate as the sodium is absorbed into the bloodstream.
Diuretics: These should be avoided If you experience sudden swelling in the face, hands, or feet, you should talk to your doctor or midwife, as this is one of the signs of pre-eclampsia (see p 89)
Cold and flu remedies: As these remedies often contain a variety of ingredients, which can include antihistamines and other decongestants that are best avoided in pregnancy it’s important to check the label carefully and talk to your doctor or pharmacist before taking any of these Try natural remedies, such as steam inhalations, before resorting to medicines, or simply take paracetamol for a short time.
Steroids: Anabolic steroids should not be used in pregnancy. It’s safe to use mild steroid creams short term for eczema, although avoid using these over a large surface area. Steroid asthma inhalers are safe, as are steroids prescribed for other conditions if your doctor knows you are pregnant.
